Default Gear are off

I have a 95 3.4L camaro, I had the transmission rebuild about 1,500 miles ago. Recently it started acting funky. Park is Reverse, Neutral is OD, Drive is 2nd and 2nd is 1st. Any ideas on what would cause this to happen?

Also make sure the nut holding the shift lever on the trans isn't loose, allowing the lever to slop back and forth. If it is loose, that'll throw off the gear selector.
