Check the plugs for broken ceramic housings. or the wires for cracks or burns.

Do the RPM's fluctuate at idle? How's your air filter and PCV? Is your throttle cable good and tight? Could be your throttle positioning sensor or a bad circuit in the MAF.

What would cause the idle to fluctuate? My idle does that but not much.

A bad IAC motor, bad TPS sensor, loose timing chain or worn timing chain gears, blown head gasket, blown intake gasket, bad fuel pump, bad oil pump.

Clogged injectors, bad spark plugs, bad wires, clogged fuel filter. Those are the main things that can cause your issue. The plugs or wires are the biggest cause of the simptoms you raise but you say those are all good.
