You still have the mono springs in the rear? Ever feel and wheel hop? I could see the rear shifting if you hit a nasty pot hole with all the weight shifted to the other side. I can not imagine the rear shifting that much with the multi leaf setup.
If you look at the stock size tires GM offered they had a fare amout of clearance between the rubber and metal.

I don't see that as likely being from suspension movement. Otherwise I'd expect you would have crushed the lip in, not pulled it out. But by all means, inspect for any shiny spots that don't belong, to indicate any suspension sliding.

Holy crap man, you're getting a lot of movement. Maybe the front eye on the leaf spring is broken? Had that before, but the rearend didn't walk forward like that.
